IF YOU HAVE NOT STARTED YOUR 2021 APPLICATION YET:
Click this link to get started on your application in the Cognito Forms system –

TO CONTINUE WORKING ON A SAVED APPLICATION:

1. Find the email sent to you by the system entitled “PAT Grant Application 2021 – Save & Resume Link”
2. Click the green “View Entry” button in that email

 

 

TO SAVE YOUR PROGRESS ON THE APPLICATION:

1. Click the “Save” button in the lower right corner of any page
2. Enter your email address
3. Click “Send”

The system will send you an email containing a link to your saved application.

To continue working on your saved application, find that email and CLICK THAT LINK.

We recommend you bookmark the link the first time you open your saved application, but that step is optional.

 

TO SUBMIT YOUR APPLICATION:
Make sure you have completed ALL of the following tasks:

1. Complete the entire application form. Required fields are marked with a red asterisk.
2. Upload the following supporting materials on page 3 of the application:

• your organization’s current 990 IRS Form (required)
• you most recent fiscal year’s audit (optional)
• your most current tax determination letter (required)
• your most recent evaluation results or findings (required if you have previously received a PAT Grant, optional if not)
• your Strategic Plan, if you have one
